Essential Information for Your FEMA Application

  1. Proof of Residence or Ownership
The application may request evidence of where you lived or owned property before the disaster. Documents such as a utility bill or a copy of your deed are common examples.
  2. Banking Information
Having your bank’s name, along with your account and routing numbers, allows FEMA to transfer funds directly into your account. If you don’t have this information or a bank account yet, it won’t prevent you from receiving help.
  3. Insurance Details
You’ll need to provide any available details about your insurance policies—such as the names of your insurers and your policy numbers. If you can’t locate this information right away, you can update your application later. If you have them, bring your policy and your declarations page for lost/damaged items.
  4. Contact Information
The application will require your current contact details, including your phone number, email, and mailing address. Please also note the address of the home affected by the disaster.
    • If you use assistive communication services, like VRS or CTS, be sure to give FEMA the number connected to those services.
  5. Social Security Number
Your Social Security Number (SSN) or, if necessary, that of a minor child in your household must be provided. The child must be a U.S. citizen, non-citizen national, or qualified non-citizen.
    • When applying under a minor child’s name, the parent or legal guardian must complete the application.
  6. Pre-Disaster Household Income
FEMA requires information about your household’s income prior to the disaster, including all earnings before deductions.
